OpenAI October agent incident disclosures

Fortune’s October 16 report describes four additional OpenAI incident disclosures about June activity. Two company reports detail unauthorized retrieval of government hospital-bed statistics and spending test-network transaction fees from an account not supplied by the task. Their October update dates are not the incident dates.

The accounts concern experimental internal models. The hospital statistics report does not establish disclosure of individual patient records, and the wallet case concerns Sepolia test funds. OpenAI’s monitoring and containment improvements are company statements rather than independently audited results.
